LOCATION/DIRECTIONS
Located in Muhlenberg Township at the northwest corner of intersection of Jefferson and Monroe Streets.
FROM ROUTE 12:
Take Fifth Street Highway North exit, proceed to Madison Avenue intersection and follow the directions below from "Fifth Street Highway".
FROM FIFTH STREET HIGHWAY:
Turn east onto Madison Avenue (Jack O'Reilly's Tuxedos on east side of intersection). At second stop sign, turn left onto Jefferson Street. Office is one block ahead on left.
FROM KUTZTOWN ROAD:
Turn west onto Monroe Street (Jack's Glass on west side of intersection). Jefferson Street is first stop sign; office on right.

APPOINTMENTS
To provide you with the most thorough care, we work by appointment only. Most patients scheduled for a complete eye examination should plan to be in our office for approximately 60 to 90 minutes. Office hours are available Monday through Thursday. Evening appointments are offered for your convenience. Contact our office at any time if you have an eye emergency, we will provide instructions for prompt care.

RECALL
We recommend that you visit our office on a regular basis. The length of time between office visits varies from patient to patient. The doctor will explain when you need to be seen again and will answer any questions that you may have.

FEES & PAYMENTS
We expect payment for all office visits at the time of the examination. If you order eyeglasses or contact lenses, you may either pay in full or make a deposit of half. All balances are to be paid in full at the time of the delivery. Our office accepts cash, personal checks, Visa, Mastercard, and Discover.

INSURANCE
We participate in many vision and medical insurance programs including Medicare, Pennsylvania Blue Shield, Capital Blue Cross, and Berkshire Health. Please ask a staff member how your insurance can be utilized at our office.

It is your responsibility to pay any deductible, coinsurance, or other fee that is not paid by your insurance company. WE CANNOT GUARANTEE PAYMENT OF YOUR INSURANCE CLAIM. Any disagreement of eligibility or coverage is between you and your insurance company.

If you wish to receive reimbursement directly from your insurance company, the receipt we give you at the end of each visit contains all the information needed for the insurance company to process your claim. We will happily complete any special insurance claim forms when required.
